A horse will only roll entirely out on their side and lie still for many minutes or even longer if they are really comfortable.   Most horses will still react to loud noises in this position, but they may need a few moments to wake up, much like us.   

Horses don’t usually lie down merely because they’re sick.  Though some horses do this when they find a particularly beautiful location to roll, things to watch out for include a horse that stands up and lies down to roll over and over.
